What Is Bipolar Disorder? 

Bipolar disorder is a brain condition that causes extreme shifts in mood, energy, and behavior. These are not normal mood changes. They are intense, prolonged episodes that disrupt a person's job, relationships, and day-to-day life.
 

There are two main kinds of episodes:
Manic episodes: very high energy, very little sleep needed, impulsive decisions, fast speech, feeling invincible
Depressive episodes: extreme low mood, fatigue, loss of interest, sometimes hopelessness or thoughts of suicide


Some people also go through 'mixed episodes' — feeling both depressed and agitated at the same time. This is one of the most painful states a person can be in.

Signs of Bipolar Disorder

Manic or Hypomanic Episode

  • Sleeping only 2–3 hours and still feeling full of energy

  • Talking very fast, jumping between topics

  • Spending unusually large amounts of money or making big impulsive decisions

  • Feeling 'on top of the world' or having an inflated sense of self

  • Increased sexual energy or taking unusual risks

  • Irritability or getting very angry when someone questions their behavior

Depressive Episode

  • Staying in bed for days, not wanting to meet anyone

  • No interest in things they used to enjoy

  • Feeling worthless, hopeless, or guilty for no clear reason

  • Changes in appetite — eating very little or eating too much

  • Thoughts of death or suicide (this is a medical emergency — call us immediately)

Types of Bipolar Disorder 

Bipolar I

At least one full manic episode. Depressive episodes are common too. This is the most severe form.

Bipolar II

Hypomania (a milder version of mania) and major depressive episodes. People with Bipolar II often get misdiagnosed as 'just depressed' because the hypomania does not look obviously dangerous.

Cyclothymia/ Rapid Cycling

Cyclothymia: A milder form with mood swings that go on for at least two years. Less severe, but still disruptive.
Rapid Cycling: Four or more mood episodes within a single year. Requires careful medication management.

What Causes Bipolar Disorder?

This is one of the first questions families ask me. The honest answer: it is not one single thing.

Is Bipolar Disorder Genetic?

Yes, genetics plays a real role. If a parent or sibling has bipolar disorder, your risk is higher than the general population. But having the gene does not mean you will definitely get the illness. Other factors matter too. I have seen patients in Indore where bipolar runs in the family, and I have seen cases where there is no family history at all.

Other Contributing Factors

  • Brain chemistry and hormonal imbalances

  • High-stress events: job loss, relationship breakdown, moving cities

  • Sleep disruption over long periods

  • Substance use (alcohol, cannabis, certain stimulants can trigger episodes)

  • Medical conditions affecting thyroid or other hormones

How a Person with Bipolar Disorder Actually Thinks

For families who feel like they cannot understand what their loved one is going through.

  • During Mania: The person genuinely believes they are at their best. They do not see a problem. Their mind is running faster than usual. Ideas feel brilliant. They feel more confident than they ever have. Telling them to slow down feels to them like someone telling a runner to stop mid-race.

  • During Depression: The person cannot remember what it felt like to be okay. Everything feels heavy. They may feel like a burden to their family. They may say things like, 'I have always been like this' even though you know that is not true.

  • In Between Episodes (IMPORTANT): Many people with bipolar disorder function completely normally between episodes. They hold jobs, raise children, and maintain relationships. Bipolar disorder is episodic, not constant. With proper treatment, the intervals between episodes can stretch to years.

Accurate Diagnosis First

Before anything else, I spend time understanding your history. How long have the episodes lasted? What triggers them? Have you used any medications before? Are there other conditions — thyroid issues, substance use, anxiety — that need to be considered? I use structured clinical interviews and, when needed, EEG and brain mapping to rule out neurological causes.

Psychotherapy — Especially CBT and DBT

Medication alone is rarely enough. Therapy gives you tools to recognize early warning signs, manage stress, improve sleep, and handle relationships better. I offer Cognitive Behavioral Therapy (CBT) for anxiety and depression and Dialectical Behavior Therapy (DBT) — both of which have strong evidence for bipolar disorder management. Therapy also helps families understand the illness and communicate better with their loved one.

Mood Stabilizer Medications

Mood stabilizers are the backbone of bipolar disorder treatment. They reduce the severity of manic and depressive episodes and help prevent relapse. The right medication — and the right dose — depends on your specific type of bipolar disorder, your age, your medical history, and your lifestyle. I do not use a one-size-fits-all approach.
